New Student Registration

KCSMA New Student Enrollment Forms

Please complete ONLY if you’ve been notified that there is a space available for your child.

What to bring?
- Original Birth Certificate with a raised seal
- Social Security Card
- Georgia Form 3300 (Eye, Ear, & Dental)
- Georgia Form 3232 (Immunization)
- Two proofs of Cobb County residency (Mortgage document, lease agreement, current utility bill, or voter
registration card)
- Custody documentation (If the child does not live with both birth or adoptive parents.)
- Any information on special services the child is currently receiving (Speech, Gifted, EIP, IEP, etc)

In order to expedite the registration process, bring copies of all documents that KCSMA may place in your child’s file. We must see the original birth certificate even if you have brought a copy and it will be returned to you after verification.
